Empowering Individuals with Safe and Stable Housing
Our Housing Access Services provide personalized support to individuals with intellectual or developmental disabilities, helping them secure and maintain safe and stable housing.
Eligibility
To qualify, individuals must:- Have a diagnosed intellectual or developmental disability
- Be approved for services through Alta California Regional Center (ACRC)
Services
Our Housing Access Services include two components:
Individual Housing Transition Services:
These services provide direct support and assistance with:
Tenant screening and housing assessments
Conducting tenant screening and housing assessments to identify preferences, barriers to tenancy, and housing retention challenges.
Creating individualized housing support plans
Creating an individualized housing support plan with measurable goals, strategies, and resources to address barriers including non-Medicaid services.
Securing resources for utility setup and essential services
Helping secure resources for utility setup (electricity, water, heating, etc.), essential services, pest control, and pre-move cleaning.
Housing application assistance and roommate selection
Assisting the individual with the housing application process. Assisting with the housing search process including roommate selection.
Coordinating resources for safe living conditions
Coordinating solutions for any potential unsafe conditions in the living environment before move-in.
Moving assistance and lease review
Supporting move logistics, including lease review, utility setup, and communication with landlords.
Developing a housing support crisis plan
Developing a housing support crisis plan that includes prevention and early intervention services when housing is jeopardized.
Individual Housing & Tenancy Sustaining Services
These services support individuals in maintaining tenancy, including:
Tenant assessment and early intervention
Early identification and intervention for behaviors jeopardizing housing (e.g., late rent, lease violations)
Housing education and training
Education and training on tenant and landlord roles, rights, and responsibilities.
Interpersonal Coaching
Interpersonal coaching for positive landlord relationships
Conflict management and dispute resolution
Assistance in resolving landlord or neighbor disputes to prevent eviction or other adverse actions.
Community resource access and advocacy
Advocacy and linkage to community resources to address housing risks.
Recertification assistance and housing support plan updates
Assisting the individual with the housing recertification process.
Ongoing tenant training and support
Ongoing training and support for lease compliance and household management.
Important Notes
- Housing Access Services do not include payment for room and board.
- Individuals receiving Health Homes or California Community Transitions services may not be eligible for this service unless additional support is necessary for their health, safety, and well-being.
