SLS Direct Service Provider

Position Description: Provide service/support to clients. Assist in goals related to Supported Living.

Openings available for a variety of shifts including: Day, swing, evenings and overnights (PT/FT and weekends).

Job Duties:

  • Supervise clients in personal care (e.g., bathing, showering, grooming, etc.).
  • Assist clients in preparing meals. 
  • Oversee clients performing daily living tasks, which may include the use and monitoring of assistive devices and precautionary equipment (i.e., Hoyer lift, bathing rails, ramps, etc.) to ensure client's safety. 
  • Perform limited housekeeping tasks in the client's personal space (not for the general household). 
  • Other duties as assigned relating to the client's direct care and supervision. 

Job Requirements:

  • Maintain all required certifications, exams, insurance, and third party affiliations/relationships as required to perform job duties (e.g., CPR certification, driver's license and appropriate insurance, IHSS affiliation, etc.). 
  • Willingness to travel to clients' home, cross train on personal care and client behavioral strategies. 
  • Ability to learn and retain new skills as required in a timely manner. 
  • Knowledge of necessary skills required for an adult to live in a supported living environment. 
  • Ability to lift 50 pounds and the strength to push and pull with a force of 50 pounds. 
  • Visual acuity to read client prescription labels and other special instructions. 
  • Cognitive and physical ability to understand and follow written or oral directions. 
  • Ability to respect the rights and dignity of all clients. 
  • Valid California driver's license with reliable transportation and proof of automobile insurance in employee's name. 
  • Willingness to work a flexible schedule, including weekends and holidays, and to change your planned schedule as emergencies arise. 
  • Employee must be able to physically perform duties assigned by any outside agency/employer that is required to maintain Assured Community Services employment. 

ILS-EPP Facilitator

Position Description: Through assessment, training, and support, facilitate self-directed outcomes for adults with developmental disabilities. Identify and strengthen the alliances, commitments, and strategies that result in unique outcomes in the lives of the clients' served.

Job Duties:        

  • Assist in determining clients' goals with regard to Title 17 "Independent Living Services."
  • Assess knowledge, skills, and abilities of clients in independent living skills (such as but not limited to: cooking, money management, shopping, etc.).
  • Provide training in areas of deficiency to meet the clients' goals. 
  • Establish training or maintenance schedules with clients. 
  • Participate in all meetings as needed to help modify training objectives. 
  • Maintain all necessary records for services rendered. 
  • Submit all paperwork to office as required. 
  • Maintain a professional relationship with external agencies, clients and their support systems, and co-workers. 
  • Immediately bring to the attention of the team and/or supervisor any opportunity for improvement in agency processes or problems affecting the agency or its services to clients; work toward resolution. 
  • Other duties as assigned. 

Job Requirements:

  • Ability to learn and retain new skills as required in a timely manner. 
  • Knowledge of necessary skills for an adult to live in an independent environment. 
  • Ability to respect the rights and dignity of all clients. 
  • Willingness to work a flexible schedule. 
  • Regular and reliable attendance is required. 
  • Valid California Driver's License with reliable transportation and proof of automobile insurance. 
  • Ability to lift and/or move up to 25 pounds
  • Ability to stand, walk, bend, sit, use hands to handle objects, lift; reach with hands and arms, climb stairs, balance, stoop, kneel, crouch or crawl, talk or hear, taste or smell
  • Vision and hearing must be sufficient to perform job functions safely including visual acuity to perform activities such as data entry, preparing reports, viewing a computer terminal, extensive reading, and visual inspection of one’s own work product at varying distances.

In-Home Respite Provider

Position Description: Provide in-home care, companionship, supervision and supportive services to Assured Community Services clients.

Job Duties:        

  • Provide companion care to include reading, visiting, answering personal correspondence, assisting with bill paying or other tasks as listed on client's service delivery plan. 
  • Provide personal care and support to assist with daily living care needs. 
  • Assist clients in preparing meals. 
  • Follow guidelines and directions established while in the client's home. 
  • Adhere to client and program confidentiality. 
  • Provide for the safety and supervision of clients at all times. 
  • Perform limited housekeeping tasks in the client's personal space. 
  • Observe mandated reporting requirements and report to a supervisor if you have concerns about a client's needs being properly met. 
  • Attend meetings as requested relating to supervision and care of clients. 
  • Maintain all necessary records for services rendered and complete daily shift notes. 
  • Submit all documents and reports to the office as required by supervisor and members of the Management Team. 
  • Maintain a supportive relationship with clients through appropriate verbal, physical and emotional responses to a client's behaviors and physical/developmental level. 
  • Maintain a professional relationship with external agencies and support systems. 
  • Immediately bring to the attention of the team and/or supervisor any opportunity for improvement in agency processes or problems affecting the agency or its services to clients; work toward resolution. 
  • Other duties as assigned. 

Job Requirements:

  • Ability to successfully pass a pre-employment background check in accordance with state and local laws and regulations. 
  • Obtain CPR and First Aid certification prior to working with ACRC clients. 
  • Ability to perform the functions required in the clients' service delivery plan. 
  • Ability to respect the rights and dignity of of all clients. 
  • Regular reliable attendance is mandatory. 
  • Personal mobile device (smartphone) for work related needs. 
  • Valid California driver's license with reliable transportation and proof of automobile insurance in employee's name. 
  • Willingness to work a flexible schedule, including weekends and holidays, and to change planned schedule as emergencies arise. 
  • Ability to lift and/or move up to 25 pounds

CFS Provider

Position Description: Provide direct support and assistance to individuals with an emphasis on client health, safety, and quality of life, oversee client Individualized Program Plans (IPP) objectives with respect to Coordinated Family Support through communication, planning, training, and implementation.

Job Duties:        

  • Providing supports necessary for clients to successfully reside in the family home.
  • Providing assistance and training to clients and their families in navigating comprehensive services and supports tailored to meet their unique needs, including creating pathways to overcoming barriers to accessing generic and other resources.
  • Providing additional information on the client’s diagnosis and identified supports.
  • Assisting with scheduling of service delivery (including medical and other appointments).
  • Identifying transportation options or services.
  • Providing future planning for clients living with aging caregivers.
  • Providing training to clients to maximize independence.
  • Coordinate the implementation of client IPP objectives.
  • Identify barriers to obtaining the IPP objectives and propose feasible solutions.
  • Complete required documentation and maintain all necessary records for services rendered.
  • Maintain all client service records, documentation with external agencies, and update the company calendar in a timely manner.
  • Display effective communication with team members, supervisor, and Service Coordinators.
  • Participate in all meetings as needed.
  • Maintain a professional relationship with external agencies and support systems.
  • Immediately bring to the attention of the team and/or supervisor any opportunity for improvement in agency processes or any problem affecting the agency or its services to clients; work toward resolution.

Job Requirements:

  • Through education and/or experience, possess the knowledge, skills, and abilities to implement all functions of the position.
  • Associate-level degree in a human services field of study or three (3) years of experience in coordinating/navigating services.
  • The ability to communicate in the primary language of the client and their family.
  • Ability to learn and retain new skills as required.
  • Ability to respect the rights and dignity of all clients.
  • Willingness to work a flexible schedule, including evenings and weekends as needed.
  • Valid California Drivers’ License with reliable transportation and proof of automobile insurance.
  • Ability to lift and/or move up to 25 pounds

TDS Facilitator

Position Description: Assistance in empowering adults with developmental disabilities by providing customized support in employment, community involvement, and post-secondary education to promote self-directed outcomes and maximize individual choices.

Job Duties:

  • Assist in determining clients’ goals with regard to Title 17 “Independent Living
    Services.”
  • Apply the TDS Program Curriculum, tailoring the program to assist clients in
    developing necessary skills required for job exploration and retention, volunteer
    opportunities, educational objectives and other endeavors he/she wishes to
    pursue.
  • Provide opportunities for clients to learn to identify their strengths and challenges,
    and develop a vision for their future, learn their rights and responsibilities and how
    to find and access resources.
  • Create an environment where clients are empowered to choose and customize
    services to meet their individual needs.
  • Establish training or maintenance schedules with clients.
  • Participate in all meetings as needed to help modify training objectives.
